PUBLIC NOTICE: Change Of Name, Formerly Called, CHINWE JUDITH NWACHUKWU But Wish To Called MRS CHINWE JUDITH UWALAKA (Copy of Affidavit Attached)

I, Mrs. Chinwe Judith Uwalaka, a native of Umuagu Agunaeze in Obowo Local Government Area of Imo State, Nigeria, an adult female and citizen of the Federal Republic of Nigeria, do hereby make an oath and solemnly declare as follows

1. That I am the deponent herein and swear to this affidavit in good faith, that I was formerly called, known, and addressed as CHINWE JUDITH NWACHUKWU.
   I now wish to be called and addressed as MRS CHINWE JUDITH UWALAKA.

The reason for this change of name is because of my marriage to CHIEF UGOCHUKWU UWALAKA.

All documents bearing my former name remain Valid, hence this affidavit.

This affidavit is now required for official/record purposes and the information of all concerned authorities to please take note.

AND THAT I make this solemn declaration conscientious believing same to be true and correct by the provisions of Oath Acts 2004
